On August 31, 2022, the emergency department of the First People's Hospital of Lin'an District, Hangzhou, Zhejiang Province received Ms. Wang and her family of four. They all developed symptoms such as nausea, vomiting, and diarrhea.

At first, they suspected that they had eaten something bad, but the symptoms lasted for a long time, so the family went to the hospital for treatment. Wu Chaoyi, the attending physician of the emergency department, received the patient and after asking about the eating history, suspected that it was mushroom poisoning.

According to Ms. Wang's recollection, the mushrooms were picked by her family from their own vegetable garden and sent to her. Because they grew in the vegetable garden, she thought they were edible, so she cooked them without worry. At dinner that day, all nine members of the family ate mushrooms, and later four of them developed the above symptoms one after another.

Because Ms. Wang's symptoms were severe, the emergency department immediately performed symptomatic treatment such as gastric lavage and fluid rehydration. The other two patients had milder symptoms and were induced to vomit under the guidance of medical staff. After blood tests, they were admitted to the gastroenterology ward of the hospital for observation. One of the children was admitted to the pediatric ward.

According to the photos provided by Ms. Wang, the suspected cause of the poisoning is the Agrocybe maxima , commonly known as the lead green mushroom. It is the most common poisonous mushroom in China with the most mushroom poisoning incidents. It can cause serious gastrointestinal symptoms, including nausea, vomiting, abdominal pain, diarrhea, etc.
